Government of Lesotho and United States of America Sign Historic Government-to-Government (G2G) Memorandum of Understanding (MOU) to Strengthen HIV/TB Services in Lesotho

The Government of the Kingdom of Lesotho and the Government of the United States of America today, the 10th of December 2025, signed the MOU, marking a historic Government-to-Government (G2G) Health Cooperation Contract aimed at strengthening and sustaining HIV/TB services across Lesotho. The agreement marks a major milestone in the long-standing bilateral partnership between the two governments in the fight against HIV/TB and in strengthening the national health system.

The MOU will ensure the continued delivery of life-saving HIV/TB prevention, treatment, and care services, while advancing Lesotho’s transition towards greater national ownership and sustainability of its HIV response. The agreement focuses on strengthening health systems, stabilising the health workforce, ensuring uninterrupted access to essential HIV/TB commodities, and enhancing national monitoring and accountability systems.

Under this agreement, funds are channeled directly to the Government of Lesotho through agreed financial management systems. Implementation is led by the Ministry of Health in collaboration with relevant government departments, development partners, and implementing partners. Clear accountability, reporting, and performance monitoring mechanisms are built into the agreement.

Lesotho remains one of the countries most affected by HIV globally, and the United States has been a key partner for decades through initiatives such as the President’s Emergency Plan for AIDS Relief (PEPFAR). This new G2G arrangement signifies a shift toward direct collaboration between the two governments, reinforcing confidence in Lesotho’s systems and leadership.

The key objectives of this agreement are to:

  • To sustain access to HIV/TB prevention, treatment, and care services.
  • To strengthen the national health system to manage and implement HIV/TB programs effectively.
  • To enhance government ownership and leadership of HIV/TB service delivery.
  • To stabilize the health workforce involved in HIV/TB service provision.
  • To improve the availability and management of essential HIV/TB commodities and medicines.
  • To reinforce monitoring, evaluation, and reporting systems for HIV/TB programs.

The signing of the Government-to-Government HIV/TB contract between the Government of Lesotho and the United States of America marks a new chapter in bilateral cooperation. It secures the future of life-saving HIV services, strengthens the national health system, improves the well-being of Basotho, and reinforces the vital role of key partners such as CHAL in achieving national health and development goals.
